A community service reporting form should be completed after any member of the ESF community completes volunteer work. If your volunteer work is ongoing, make sure to keep documentation of your hours and complete one form at the end of each semester or when the work is completed. The community organization may be contacted to verify the information you provided is accurate. This form will not be accepted and recorded unless it is completely filled out.
