Participating in an ESF International Faculty-Led Program/Course
Office of International Education

ESF faculty/staff who plan to conduct an international program or academic course abroad must obtain departmental approval and adhere to all ESF and SUNY policies and procedures regarding international travel and student programs.


Initial Planning: Departmental Approval and Notification of Your Intention to Plan an International Program/Course

Initial planning for an international program or course should begin at least a year in advance of the proposed travel and should include attention to the curriculum for the course as well as travel logistics (entry requirements, immunizations, travel alerts, reservations, and transportation). Please consult your department chairperson to determine the steps necessary to propose and obtain departmental approval of an international ESF faculty-led program/ course. Once you have obtained departmental approval, you must submit the Proposed ESF International Program Form to the Office of International Education. This form will serve as official notification of your intention to offer an international program course. Please update the Office of International Education as your plans develop/change.

Faculty program directors must submit the following to the Office of International Education as soon as departmental approval is received:

  • Proposed ESF International Program Form


Continued Planning: Program Director and Student Paperwork to be Submitted No Later than One Month Prior to Travel


Program Directors:

  • International Program Itinerary Form
  • A copy of the faculty program director(s)’ passport photo/identification page and passport expiration page
    (An international traveler's passport should be valid at least 6 months beyond the anticipated date of re-entry to the U.S.)
  • A copy of the faculty program director(s)’ visa (if applicable)
  • A copy of the faculty program director(s)’ flight itinerary
  • An excel spreadsheet of participant information that includes each participant's SU ID #, name, date of birth, and email address
  • A completed packet of International ESF Faculty-Led Program Forms for each student who will participate in the program/course including the required  supplemental documentation

Faculty program directors should ensure that each student who will participant in their ESF faculty-led program/course abroad submits the documentation noted below. Faculty should submit forms to the Office of International Education in a complete packet.


Students:

  • Completed pack of International ESF Program Forms
    (Participant Information; Agreement and Release; Student Health Information; Physician’s Statement; and Statement of Insurance)

    Students are REQUIRED to comply with college policy for all international programs including submitting the complete packet of international program forms with supplemental documentation. If students do not comply, they will be "unregistered" for the associated coursework and will not receive credit.

  •  A copy of your passport photo/identification page and passport expiration page
    (An international traveler's passport should be valid at least 6 months beyond the anticipated date of re-entry to the U.S.)
  • A copy of your visa (if applicable)
  • A copy of your flight itinerary (if you will be flying separate from the faculty program director or no group flight was arranged)
  • A copy of your international health insurance card/plan
  • A copy of your Medical Evacuation and Repatriation Coverage card/ plan if it is not included in the general international health insurance coverage

If you do not have comprehensive health insurance with medical evacuation and repatriation, you may purchase the SUNY plan at the Cashier's Office for $83/ month. If you do not have medical evacuation and repatriation insurance, you may purchase it at the Cashier's Office for $6 /month.
