Tutoring Applications & Forms

To apply for an individual/small group tutor:

1. Complete a Request for a Tutor Form. Forms are available in 109 Moon Library, online, or download the form. (PDF—viewable with Acrobat Reader). Please fill out the application completely.
2. In not submitted online, return the form to Rm. 109A in the Moon Library.
3. The Tutoring Coordinator will respond to your request via e-mail from tutoring@esf.edu within a couple of days. If you do not receive a response within 72 hours, please e-mail tutoring@esf.edu.

To become a peer tutor:

1. Complete a Peer Tutor Application Form. Forms are available in 109 Moon Library, online, or download the form. (PDF—viewable with Acrobat Reader). Please fill out the application completely.
2. In not submitted online, return the form to Rm. 109A in the Moon Library.
3. The Tutoring Coordinator will respond to your application via e-mail from tutoring@esf.edu within a couple of days. If you are eligible to become a Peer Tutor, the Tutor Coordinator will schedule a tutor training time with you to go over all the tutoring procedures and paperwork. If you do not receive a response within 72 hours, please e-mail tutoring@esf.edu.
