MMR and COVID-19 Waiver/Exemption Information

New York State law, SUNY policy, and Syracuse University policy direct the MMR and COVID-19 immunization and vaccination requirements for SUNY ESF students. Any student who believes they should be exempted from either/both the MMR and COVID-19 requirements must apply for and be granted an exemption. Students may request the following exemptions or accommodation:

  1. Online or Fully Remote Exemption

  2. Medical Accommodation (this is required if you have not yet received your final dose of vaccine before arriving on campus)

  3. Medical Exemption

  4. Religious Exemption

To request exemption from the MMR immunization, complete the 2nd page of the MMR and Immunization History Form and Response and return it to the Division of Student Affairs via email to studenthelp@esf.edu. Please note any requirements for additional documentation.

To request exemption from the COVID-19 vaccination, please complete the appropriate form and return to the Division of Student Affairs via email to covidsa@esf.edu.

All exemption requests will be reviewed by the College's Exemption Request Committee. Please allow 2-3 weeks for a response to your request. Questions can be directed to the Division of Student Affairs via email at studenthelp@esf.edu or by calling 315-470-6660.
