General Immunization Information

New York State law, SUNY policy, and Syracuse University policy direct the immunization requirements for SUNY ESF students.

COVID-19 Info

SUNY policy 3100 requires all students accessing SUNY facilities in-person to receive COVID-19 vaccination series. A temporary accommodation will be made for students who have started the vaccination series but need additional time to complete the series.

In addition, Syracuse University requires individuals accessing any SU campus facilities, including classrooms, dining halls, libraries, and recreational facilities, to receive COVID-19 vaccination series, plus one booster dose upon eligibility.

Required - submit COVID vaccination via the SUNY vaccination portal

Deadline - must be completed prior to arrival on campus

Exemption/Waiver Info:

Students may request the following exemptions or accommodations:

  1. Online or Fully Remote Exemption

  2. Medical Accommodation (this is required if you have not yet received your final dose of vaccine before arriving on campus)

  3. Medical Exemption

  4. Religious Exemption

Completed Exemption requests should be submitted to covidsa@esf.edu as soon as possible to allow time to process prior to arrival on campus.


MMR & Meningitis Info

Due September 25 (for students admitted for the fall semester)
Due January 15 (for students admitted for the spring semester)

Required - submit two completed forms via MyESF student portal

Deadline - must be completed prior to arrival on campus

Pursuant to NYS Public Health Law § 2167, all students born on or after January 1, 1957 are required to receive MMR (measles, mumps, and rubella) vaccination and the College is required to collect this information. Additionally, all students regardless of age must read and certify they have been informed about meningitis.

Any student who believes they should be exempt from the MMR and meningitis immunization requirements must follow the directions on page 2 of the MMR and Immunization History Form.

Failure to submit these forms by the identified deadline may result in your class schedule being cancelled.

Flu Vaccine Info

Required annually to access Syracuse University resources and facilities such classrooms, labs, dining facilities, libraries, and recreational facilities.

Flu vaccine document submission will be available soon.
