Ordering a Duplicate Diploma

To request a duplicate diploma the following information is required:

• Your name
• Your social security number
• Your birth date
• Complete address where you would like the diploma sent
• Your handwritten signature
• A check in the amount of $30.00 payable to SUNY-ESF
