Change of Major Forms
The Change of Major form is required for all undergraduate student major changes. Depending on the major you are changing to, you may be required to complete the Change of Major form AND the Student Attestation Form for Professional Licensure Programs. Please see below for instructions.
Change of Major Form - Required for All Majors
Additional Requirements for Major Changes to Landscape Architecture, Chemical Engineering, Bioprocess Engineering, Environmental Resources Engineering, or Paper Engineering Programs:
Many professions and certifications are governed by state or territorial regulations. These regulations often dictate specific educational requirements for licensure or certification. For this reason, some of SUNY ESF programs or courses that lead to professional licensure and/or certification in New York State may not meet the educational requirements of all states and territories. The attestation process ensures that students engage in the decision-making process with full awareness and consent.
Students requesting to change from their current major to the Landscape Architecture, Chemical Engineering, Bioprocess Engineering, Environmental Resources Engineering, or Paper Engineering majors are required to:
- Meet with a department representative (curriculum coordinator or department chair) from the new program to discuss professional licensure and location.
- Complete and sign the Student Attestation Form for Professional Licensure Programs
- Submit both the completed Change of Major Form and the Student Attestation Form for Professional Licensure Programs to the Office of Admissions in 218 Gateway Center.
